1Sa 21:10-15 At Gath He Feigns Himself Mad.

David fled to Achish, the king of Gath, one of the five main Philistine territories. Given his reputation as their greatest enemy and the presence of Goliath's sword in his hand, this was a perilous move. However, it seems that David had received divine guidance, which likely ensured his protection.

Achish was known for his generosity (1Sa 27:6), and he may have seen an opportunity to weaken Saul's resources by harboring David. This practice of great men seeking refuge with neighboring princes was common in ancient times.

David pretended to be insane, possibly suffering from epilepsy or feigning the condition. In this state, he would often foam at the mouth. His behavior was so convincing that Achish thought him truly mad when David let his spittle fall down onto his beard – a grave insult in Eastern culture, especially when done by one's own hand.
